Breast augmentation

Breast augmentation is to enlarge the breasts by putting silicone implants to women whose breasts are small by birth or shrunken later. In addition to silicone, it’s also possible to enlarge the breast by fat injection.

When breast augmentation surgery is done:

It can be done to every healthy and willing woman who is over 18. Breast enlargement is not performed during pregnancy and lactation. At least 3 months should pass once the lactation is finished. It can be made after being sure that no milk is coming from the breasts. Surgery can be done in all seasons.

Information about silicone implants:

Breast implants briefly known as silicone, are divided into two groups as round and anatomic. Round implants are circular, anatomic (teardrop) implant as the name implies, has a longer up – down diameter, so is in the shape of a teardrop. The reason for being named anatomic is that it’s much more similar to the original shape of the breast. The silicone implants are composed of a silicone solid cover which is filled with silicone gel. According to the surface structure, the implants are divided into two groups as smooth and textured surface. For breast augmentation, it’s always necessary to use quality, FDA approved materials.

Breast augmentation implants have lifetime warranty. They can be used unless there’s a deterioration in the breast tissue, and does not need to change. Ultrasound and MRI which is applied every year can detect the silicone. Having breast augmentation before or after pregnancy doesn’t matter. Silicone doesn’t interfere with lactation.

It’s possible for a person to have any size breast implant that she desired. But the healthiest is to have the appropriate size to your chest and shoulder size and your height. If you prefer a silicone size far exceeding these dimensions, the image won’t be natural and the silicone will be visible and it can even cause ondulation on the skin.

If the the breast tissue is sagging too much, than only breast augmentation with silicone won’t be enough. Breast lifting surgery must be performed on the same session.

Another methods of breast augmentation are using filler or fat injection. Fat injection can be applied to women that have some amount of breast and want to plump it. It‘s also essential to have the amount of fat to be used in the transfer in the body.

Operating information:

As breast augmentation surgery is done with general anesthesia, it’s absolutely done in a full-fledged hospital. Operating room conditions, general hospital hygiene, patient care, hospital staff and equipment should be very good.

With hypodermic incision, we get underneath the breast There’re methods that we call submuscular, inframammary, or dual plane. We decide which method to choose based on factors such as the patient’s height, weight, and shoulder width. Incision is made underneath the breast. Silicone is placed from the area where the incision is made. It’s ensured that the breasts are symmetrical.

Breast augmentation surgery takes 1-2 hours. You are allowed to go home on the day of surgery or the next day. The first two days should usually be spent resting. In this period, depending on the placement of the prosthesis behind or in front of the muscle, you may have different complaints. Generally, in prostheses placed behind the muscles, pain is increased with arm movements. This situation is limited limited to the first days. Edema (swelling) increasing in the first two days begins to decrease from the third day. Arm and body relax. On the third or fourth day of the breast augmentation surgery, bands in your chest are removed. Only heat-resistant bands placed on the seams are left. You are allowed to take shower with these. With the sports bra that will be used for one month after the surgery, the shape of the breast is provided.

The activities after breast prosthesis (breast silicone) implementations, should be strictly restricted for the first three weeks, gradually increased after this period. It’s not allowed to play tennis and do heavy sports before the first month. However, walking can be done. During the 6-8 weeks following the breast augmentation surgery, sauna, solarium and steam bath should be avoided.

Those who do desk jobs may go to work after 3 days.  However, for those who have jobs requiring arm power, it’s appropriate to take a break for about a week.

The Risks of Breast Augmentation:

  1. First of all, this surgery should be performed on healthy people. Doing a breast augmentation surgery to someone whose health is unsuitable for the surgery, will put a person’s life at risk. Therefore, we try to detect these risks with a number of pre-operative tests, and especially with the examination of our anesthesiologist. The important issue here is: you should also tell us about your existing ailments and medications you use.
  2. Anesthesia-induced complications
  3. Infection
  4. Capsular contracture
  5. Bleeding

Rhinoplasty

Rhinoplasty surgery is the process of correcting the nose structure that’s unpleasant to image by surgical methods to make person’s liking. The nose, in terms of function, is the organ that allows the breath we take to enter the body in a healthy way.

Congenital or post-traumatic nasal deformities can only be corrected by rhinoplasty surgery. We aren’t only correcting the image while doing the surgery, we provide clarification of the airways for healthy breathing.

General Rules of Rhinoplasty;

  • Rhinoplasty is done by aesthetic and plastic surgery consultants.
  • If the patient doesn’t have a request like having an especially obvious one, making a transaction compatible with the patient’s face, not revealing the aesthetic,
  • Not to disrupt the bone structure that’s tectorial for nose’s breathing function, not to create unnecessary trauma, to increase breathing function while shaping,

What should I pay attention to before nose surgery (Rhinoplasty)?

You must be completely healthy. Meaning, there shouldn’t be a situation such as flu. The use of aspirin and similar drugs should be quitted a week before. If you’re wearing glasses, you need to provide corrective lenses. Because, it’s prohibited to use glasses for at least 3 months after the surgery. If you smoke, reduce as much as possible in the weeks before surgery, and quit even if possible. It would be appropriate to cut alcohol use a few days before.

In general, we can mention two types of Rhinoplasty:

The first is the closed technique; the main difference from the other technique is that it’s done in the region between the nostrils called columella, without any skin incision. In this technique, cartilage shaping can be done by delivering the cartilage out of the nostril. All of the rhinoplasty procedure done in the hump section, are carried out gropingly, without being seen clearly.

The second is the open technique; through a small skin incision made in the columella region, the nose tip and dorsal line can be made completely visible. Thus, the processes on the dorsal line can be made more sanitary. Rhinoplasty can be made very easily by correcting stenoses causing curvatures and shortness of breath, with cartilage supports. Shaping of the nasal cartilage is done visually, and thus a more proportional appearance is provided.

What do I experience after nose surgery (Rhinoplasty)?

Rhinoplasty surgery takes about 2 hours. For 4 hours immediately after the surgery, side effects such as fatigue, drowsiness and nausea caused by anesthesia occur. There’s no pain in the nose, because we numb the nose with a long-acting painkiller. Nonetheless, if the tenderness in the surrounding tissues disturbs, they also respond to the given painkillers. Nose bleeds slightly on the day and the next day, mixed with mucus. This is a normal process. It’s very important to regularly perform ice treatment on the day of rhinoplasty. You are discharged the day after the surgery. It’s important to continue the ice treatment, and keep the nose tip clean at home. You should use the medications given to you regularly and protect your nose from trauma. No water should touch your nose. For this reason, you can have your hair washed in the barber / hairdresser. Some patients are more likely to have bruises and swelling.

A week after your rhinoplasty surgery, splints and silicone supports on top of the nose are removed.  Sutures are removed, if there’s any. The nose gets taped again. After a week from this date, the nose is fully opened and massage therapy begins.

After rhinoplasty, maximum of 1 week rest is sufficient. If you are doing a job related to sports, you can only do non-sport activities for 1 month.

There’s very little possibility of indentation after rhinoplasty. Because, the majority of surgical procedures are performed through the nose.

Breast Reduction (Reduction Mammoplasty)

It’s an operation that busty ladies are considering to have not only for visual intents, but also for health purposes. Because, big breasts can cause health problems. The cause of large breasts is most often genetics. Large breasts grow more and droop for reasons like weight gain, giving birth and breastfeeding. Some hormonal treatments can also lead to breast enlargement.

Health problems caused by large breasts:

  • It can lead to cervical disc hernia, can cause neck, back, shoulder and even low-back pain
  • Rashes can occur due to sweating and perspiration under inframammary line, makes bad odor in this area
  • Difficulty doing sports, and trouble finding clothes
  • Cavitations created by bra straps on both shoulders that have to carry heavy breasts
  • Psychological distress caused by fat appearance

Operation procedure:

Breast reduction surgery takes about 2 hours. The length of stay in hospital is 1 day. There’s not much of pain. Standard painkillers are sufficient. The antibiotics etc. given by the doctor should be used regularly. Shower can be taken on the 7th day after surgery. Generally a week of rest after surgery is enough. Controls are done in the first and second weeks.  The second week, after stitches are removed, scar treatment is continued. Since sports bra is used, patient feels safe, and can working.

Don’t force your arm for at least 4 weeks after surgery, do not carry weight, avoid push-pull movements. You can walk, but heavy sports is prohibited for 1 month. You should use the sports bra recommended to you by your doctor continuously for a month. This bra will reduce pain, as well as help shape the breast.

Don’t smoke for at least a month after surgery. Smoking, impairs wound healing, also by narrowing smaller vessels, reducing blood robust of tissue and skin in the surgical area, disrupts tissue perfusion; can cause such situations such as cell damage, cell death, wound opening, suture opening, inflammation. It can lead to worsening of scars that normally will heal properly.

What are the risks of breast reduction?

  • bleeding
  • congestion in tissue (hematoma)
  • significant scarring (hypertrophic scar-keloid)
  • slight risk of breastfeeding problem
  • infection
  • fat necrosis, so
  • loss of the nipple

Gynecomastia

In males, breast being larger than it should be, is called gynecomastia. Males who have excess weight in puberty are more likely to develop gynecomastia. The young with gynecomastia try to hide this by wearing baggy clothes, slouching. Especially in summer, they don’t even swim just to hide it.

Although gynecomastia usually comes up as described above, may sometimes occur due to hormonal, tumoral causes (testicular tumor), or drugs used; as unilaterally (one sided) or bilaterally (both breasts).

Gynecomastia is divided into 4 types according to it’s size and drooping in skin:

Type 1: size is limited, no looseness or sagging skin.

Type 2: breast is like a small female breast, but no sagging has occurred.

Type 3: there’s a looseness that may require surgical treatment.

Type 4: breast is like a real female breast.

From what age Gynecomastia can be performed?

Gynecomastia surgery can be done from the age of 17, with the completion of puberty. But, if there isn’t much psychological distress, as it applies for all cosmetic surgeries, it’s better to expect to be 18 years old.

How is gynecomastia treated?

Only mild cases of liposuction can be operated with local anesthesia using sedative drugs. General anesthesia is preferred in patients who are planned reduction in the mammary gland, since the operation time will be longer. The operation is completed between 50 and 90 minutes.

What should be considered before Gynecomastia?

In the pre-gynecomastia period, aspirin shouldn’t be taken for 10 days, some laboratory controls are required. In addition, if there’re significant ailments in patient’s history, and medicines he/she uses constantly, they should be reported to the doctor. Smoking is an undesirable behavior in gynecomastia surgery, as it’s in all surgeries. It affects wound healing negatively. The night before surgery, after 00:00 (midnight), do not eat or drink anything.

What should be considered after surgery?

Patients, who underwent gynecomastomy under local anesthesia, on the same day; patients who underwent surgery under general anesthesia may go to their home the next day.

Compression vest is begin to be worn immediately after the surgery.  The use of this vest is pretty easy. The vest should be worn continuously for three weeks. After the third week, it’s useful to wear the vest only during daytime, especially while doing heavy activities. This period is about 6 weeks.

In the first days, edema, sensitivity, pain, even bruises may occur in the operated area. These usually begin to gradually lessen after the first 2-3 days. Sensitivity may last for weeks. Although not in size to complicate daily life, numbness, stinging and burning may appear as patient complaints. After the second week, it’s allowed to enter the pool and sea. Also for 6 weeks, it’s tried to avoid the factors that may lead to edema, like steam bath, sauna, solarium. Likewise, making heavy sports, and activities that might give blow to the chest area, should be avoided for 6 weeks.

If the person doesn’t get too much weight, there’s no possibility of reoccurrence of gynecomastia. Some subsequently received anabolic drugs, excessive alcohol use, or some diseases rarely cause gynecomastia to recur. Aging, and along with excessive weight gain also increase the risk.

What are the risks of Gynecomastia?

-Bleeding

-Hematoma

-Infection

-Irregularities in the rib cage

-Long-lasting numbness, tingling and color changes can be seen. These usually recover over time.

Brazilian butt lift

Slim and flat-bottomed ladies desire their buttocks to be more specific, especially when wearing pants. Since it’s not possible to obtain enough fat from thin people with liposuction, the bottom is shaped with silicone implants.

In people who have serious weight gain/loss, flattening occurs in the buttocks. In this case, which is accompanied by sagging in the abdomen, hips and inner legs; abdomen, buttocks, legs and buttocks are stretched together. Traces are hidden in the bikini line at the waist.

Sometimes, due to genetic features, the angle that the bottom makes with the waist is wide. The bottom is very vague when viewed from the side. However, there’re enough fat in the hips. Fats obtained from buttocks by liposuction method, can be used for shaping the butt.

Is bottom shaping with fat injection permanent?

Although it’s a natural and more preferred method, some of the fat tissue is lost after about 1-1.5 years. But if there’s a reserve, the transaction can be repeated. With some techniques developed during studies on fat injection in recent years, have increased the possibility of fat cells to be permanent. Butt augmentation with fat injection is performed under general anesthesia, and the duration is 1-1.5 hours. After surgery, special corset is worn for 2 weeks. Can return to work after 3 days.

Butt augmentation with filler

Aesthetic technology has gained a rising momentum. With different filling materials for each region of the body, it became possible to be beautiful. We use these fillings to round and shape the buttock area.

In recent years, Kim Kardashian bottom also began to be demanded. With fat injection and filler combination, we can give a beautiful shape by enlarging the buttocks toward the sides or back. If breasts are also in the desired fullness, you’re able to have a quite curled body.

Labioplasty and vaginoplasty

The small lips (labia minora) that are part of the external genital organ are genetically larger and longer in some people. It’s also called as inner lip sagging. It’s even more disturbing, especially for those who are thin. Lips that are noticed from tight trousers and swimsuits, also affect the clothing options of the person.

Reduction of the inner lips can be accomplished in about 30 minutes under local anesthesia. Desiring delicate patients, can also be treated under general anesthesia. Once the small lips are numbed, the inner lips can be evenly reduced so that they do not sag too much between the outer lips. Repair is completed with dissolving sutures.

Information is given on how to keep the area clean. Antibiotics and pain relievers are given to use at home. They can return to work the next day. Can take a shower after 24 hours. For a day or two, putting a pad in the underwear provides comfort to the person. About a week, sports activities and using bathtub should be avoided. Sutures fall within 2 to 3 weeks. After that, you can return to your sexual life.

Small lips don’t grow back again. There‘s usually no obvious mark left in the surgical area. But sometimes, wound healing may be more in some people. This may cause sensitivity in this region. In such cases, the use of certain topical creams, or correction of traces may be required.

Vaginoplasty

Vaginal tightening surgery is most often done, as the vagina can’t provide adequate grip during sexual intercourse, and sound coming from the vagina, the sexual intercourse is negatively affected, as a result of loosening of the vaginal muscles in women given natural birth. As the number of labours increases, this becomes even more obvious. In older ages may go up to urinary incontinence, urinary bag and large intestine sagging, both together, and even the uterus sagging.

How is vaginoplasty surgery done? How long does it take?

Usually, it’s an operation performed under general anesthesia due to its psychological dimension. With the epidural anesthesia (numbing the waist) procedure, a comfortable operation can be performed. The process takes 30-45 minutes. The most preferred method is the constricting process made from the posterior wall of the vagina.

After vaginoplasty, you can exit the hospital the same day. The tampon placed into the vagina after vaginoplasty is pulled the next day. No medical dressing is required. No suture removal is required. Sutures resolve themselves. Only, using the antibiotics given, and paying attention to the toilet cleanliness is sufficent. For 1-2 days, a pad can be placed in underwear. The most important point is to avoid sexual intercourse for 6 weeks after vaginal tightening surgery. It’s also beneficial to use vaginal drawing compounds called “lubricants”, during the first sexual intercourse after the surgery. After vaginoplasty, sexual life is expected to be better.

What are the complications that may occur after surgery?

If 6 weeks of sexual intercourse ban is followed, the probability of encountering a problem is very low. Early sexual intercourse may cause re-expansion, due to compression applied to the tissue, without the wound reaching a sufficient strength.

Plumping big lips

Big lips may look wrinkled and saggy in some people. As this may be a genetic feature, it’s a condition mostly we come across after 30 years of age or very thin people. In the desired external genital appearance, the outer lips should be of sufficient fullness, and cover the small lips to a large extent.

The first choice for filling outer lips is should be the person’s own fat. Sometimes people may want to have this procedure as an outpatient treatment in the clinic, fillers containing large particulate hyaluronic acid may then be used.

Fat injection is performed under operating room conditions, by local or general anesthesia. Generally, the fat tissue taken from the abdominal region is injected into big lips. The patient may go home after resting for several hours. No dressing and suture removal are required.

After filling treatment, to be more in fat injection, swelling in the lips occurs in both injection methods. The swelling gradually decreases within a few days. During this period, wearing more baggy underwear decreases sensitivity a little bit. After surgery, antibiotics and pain relievers are used.

The only problem that may occur is infection. This can be treated with antibiotherapy.

Abdominoplasty (Tummy Tuck)

Abdominal skin sagging that requires abdominoplasty occurs usually in women. However, as a result of excessive weight gain in men, sagging and loosening occurs due to deformation in the abdomen, belly skin and muscles. The most important factor in women is past pregnancies. Significant weight loss also causes the abdomen to loosen and sag. With abdominoplasty, the frontal wall of the abdomen, which is enlarged after pregnancy is narrowed, relaxed and separated abdomen muscles are tightened, the excess of sagging abdominal skin taken together with the fat.

Abdominoplasty may take 2-5 hours depending on the procedure. Often, an incision line is made 8-10 cm below the navel and over the pubic hair area, which can be hidden under the linen. Think of it as a little longer than caesarean section. A second incision is made around the belly for the belly-button transfer. When the surgery is over, the incision line is closed under the skin with hidden sutures and covered with dressings. One or two tubes, called drains, are placed in order to take out excess fluid to be collected in the region. Immediately after the abdominoplasty, the person is dressed in appropriate corset in the operating room.

What should be considered before abdominoplasty?

If you smoke, you should give it a break 2 weeks before and 2 weeks after the surgery. If you use vitamin E and aspirin, take a break during in period, and avoid heavy diets before surgery. Cold, or another infection, may cause surgery to be postponed.

When can I have abdominoplasty after giving birth?

The ideal time for mothers to have abdominoplasty is the end of the first year. During this time, the mother loses the weight she can, as well as the degree of sagging and cracks are fully revealed. Abdominoplasty also doesn’t pose a problem for later pregnancies. However, if a new pregnancy is planned very recently, it’s still better to postpone this surgery.

When is the most accurate time to have surgery for those having deformation due to weight loss/gain?

It’s better to do your abdominoplasty at the end of the weight loss process. Thus, it’s possible to see and treat the entire deformation. However, sometimes supporting patients with surgery who noted a certain stage during weight loss process, increases adaptation to the weight loss process.

Your abdominoplasty scar can be easily hidden in underwear. The thin, pink color line will gradually become white. By using trace creams and silicone bands recommended by your doctor, you can make your abdominoplasty trace as less obvious as possible.

In the first few days after abdominoplasty, the abdominal area will be swollen. Also, having the pain and discomfort that can be relieved by taking medication is also normal. Drains placed during surgery, are usually removed on the 3rd -5th days. Depending on the surgical procedure performed, you stay in the hospital for one, rarely for a few days.

It may take several weeks to return to old activities after abdominoplasty. If the physical condition of the patient is good and has strong abdominal muscles, this process takes even shorter. Exercising will positively affect the healing process, help to reduce the amount of swelling, reduce the risk of throwing blood clots, and recover abdominal muscle tone. However, strict exercise programs, especially those that force the abdominal muscles, are postponed until they can be easily done.

What are the risks of abdominoplasty?

– infection

– accumulation of blood or body fluid in the operative field

– blood clot break off

– bad wound healing

Blood clot break off (emboly) status is a complication that should be kept in mind especially in elderly patients, may jeopardize life, and may be seen after every surgery. This risk is usually remedied by rousing up early and walking the patient after abdominoplasty, using compression socks, and certain medicines.

Liposuction

The process of absorbing apparently excess and undesirable fats in the body by a medical device with suction function over a special, hollow cannula, is called liposuction.

It’s performed under general anesthesia. Therefore,, the patient doesn’t feel anything. Sometimes, fat removal operation of very small areas can be done by local anesthesia. It’s not a surgery to lose weight as it’s supposed to, but an operation for shaping the body by taking the remaining fat of the people who lost weight. Provides a more pleasing appearance by fulfilling the deformities of people who have only a little to achieve ideal weight.

The most common situations in which liposuction process is applied, counted as follows:

  • To fix these problems of adults of normal weight and height with fat deposits (especially the side of the hip, the belly, inner knees, love handle in the side of waist, etc.) leading to regional deformity in the body,
  • In young men, to treat the discomfort manifesting itself in the form of breast enlargement, and called gynecomastia,
  • To enable the region to be shapened better, and to retouch after neck lift, tummy tuck and breast reduction surgeries.

It’s reasonable to have liposuction after doing hip loosening exercises with the help of a sports instructor to get rid of the excess especially in the hip region. Belly fat burning movements will also burn the belly fat, make it ready for fat removal.

  • Abdomen and waist circumference (belly)
  • Inner and outer sides of thigh
  • Sides of knees, facing each other
  • Upper arm

Less preferred regions for fat removal:

They’re usually the cases where the doctor and the patient decide together. Less preferred or non-preferred body regions in this method are counted as;

  • parts below the knees in legs (should be done especially with thin cannulas)
  • gluteal area (buttocks) (May be preferred if there’s excessive protrusion and asymmetry in the gluteal area.)

Liposuction in men:

In Turkey, the number of men having liposuction is gradually increasing. Liposuction, after rhinoplasty, is the second most common cosmetic surgery that men have. Men are usually applying for fats in their belly area.

Vaser Liposuction

In Vaser liposuction system, first a small amount of fluid is injected into subcutaneous fatty tissue. Ultrasound energy is given to this fluid injected area by a special device. Thus, this ultrasonic energy is being spread with fluid between the fat cells. Fat cells are separated from other nerves, vessels and connective tissue by shaking up. After that, it’s being aspirated. Also in conventional liposuction, we inject fluid between the tissues. With the fluid, fat cells are separated a little, but by using our own strength we draw that cells from there. It’s said in Vaser liposuction, because the ultrasound energy separates the cells, larger amount of fat can be removed with giving less damage to other structures. There’s information such as that it’s more effective in loss of elasticity of the skin, better at gathering the skin, but these aren’t conclusive findings.

Body-jet method

Especially, if it’s the case of transferring removed fat to another region, the most convenient method for fat removal is Body-jet method. In fact, this method takes its name from the brand of the device used. The working method is the removal of the fat by flushing pressurized water to the fat removal area, with the help of this device. Following this, removed fats are taken into the device, separated from the fibrous tissue and the external elements other than fat. Thus, a rich material is obtained also from the stem cell. The transfer made with this material is thought to be more long-lasting in the body. However, it isn’t very practical to use it only for liposuction. Both operation takes a long time and costs more.

Is liposuction harmful?

Liposuction isn’t considered a risky operation, unless the person has a serious illness such as heart problem, hypertension or diabetes. The risk of anesthesia is also low, because it’s applied to near-normal weight, relatively young (under 60 years old) patients.

Complications encountered in all surgeries:

Some small outcomes, usually seen in every operation, can also be seen after this procedure. To summarize these briefly;

– Complications due to anesthesia procedure (drug allergy…)

– Bleeding, hematoma (subcutaneous blood congestion)

-Infection

-Systemic complications may be seen in those with chronic heart, lung disorders, diabetics

-Emboly (Fat embolism is a complication most often seen in long bone fractures.)

Complications may be seen after fat removal surgery:

  • Waving on the skin surface, undulation (Prevented by various massage or lpg device application methods starting 3 weeks after surgery.)
  • Loosening due to insufficient self-recovery of the skin (It’s a condition observed in the abdominal region of women whom are especially in the post-menopausal period and have given birth At the earliest 6 months after the liposuction procedure, it’s possible to achieve a more aesthetic appearance by performing surgeries, called as tummy-tuck (abdominoplasty), thighplasty which are performed as removing fat with skin surgically)
  • Skin loss due to insufficient operating of veins (Seen rare. Particularly in smokers, liposuction of the neck area process should be done very carefully.)
  • Surficial sensation loss (Usually temporary)
  • Skin burn (A rare complication due to heat, which also occurs in ultrasonic liposuction.)
  • Perforation of the abdominal wall (It may occur during abdominal area fat removal, especially in women given many births, patients with hernia. Pre-operative diagnosis of such patients is very important.)

Again and consequently, if your height-to-weight ratio is 20 units or less (eg 160 cm – 80 kg) or the body mass index is at most 65, you can also be freed of your excess fat that you cannot get rid of by calling us.

Blepharoplasty (Eyelid surgery)

At 30s, the downward displacement begins in forehead and eyebrows. This’s manifested by the loosening of upper eyelid skin. At 50s, with the thinning of the muscles in the upper eyelid, both the loosening in the skin increases, and the fat pads in the eye socket become evident.

In the lower eyelid, at 40’s, fatty tissue in cheek displaces downward under the effect of gravity. This situation is manifested as the halo deposit under the lower eyelid, and the appearance of smile lines on the edge of the mouth. With the thinning of the lower eyelid skin, the lines that we call crow’s feet begin to form around the lower eyelid. With the weakening of the lower eyelid muscles at 50s, the fat pads in the eyeball become more prominent as they lose support in front of them.

Do not use aspirin or similar medication for a week before surgery. Likewise, since it’s believed that herbal teas such as green tea and vitamin E have blood-thinning effect, they should also be avoided for a week before surgery. Discomforts such as excessive eye pressure, hypertension, thyroid diseases, eye dryness are the conditions that increase the risk of eyelid surgery. If you have one of these chronic conditions, should inform your doctor about it.

If only upper eyelid surgery is to be done, operation can be performed under local anesthesia. Upper eyelid surgery duration does not exceed an hour. The amount of skin and muscle to be removed is decided by the drawing done while the patient is sitting. Often, removal of the skin and muscle in appropriate sizes to cause a line to be hidden in the folding line of the lid provides sufficient tension on the eyelid. But in some cases, the fat pads that are available in the interior and the middle may be very noticeable. Therefore, in a small amount, their reduction is made surgically.

It’s necessary to evaluate the aging of the lower eyelid with aging at the cheek. Often, at 40s, cheek tissue is displaced downward, the cheek tissue should also be lifted by endoscopic surgery. After this procedure, it’s decided that the lower eyelid aesthetic should also be performed during the operation if there’s still plenty of lower eyelid skin sagging and there’s a clearance in the lower eyelid fat pads.

The incision made just below the lower eyelid lashes and the lower eyelid skin is removed with the muscle. Minimizing the fat pad is required very rare. Typically, spreading of fat packs to the edge of the eye helps correcting the collapse formed under the eye. In adjustment of this collapse, the fat injection procedure is also added if cheek lifting and spreading of the fat pads are inadequate. Once the skin and muscle abundance are removed, muscle tissue is hung on the outer wall of the eye socket. This also provides support for fat pads.

When the process is over, there will be bands covering the eyelids. These, doesn’t interrupt your vision. Cold application is performed to the eye contour to control edema after treatment. Despite these precautions, swelling and bruising can occur around the eye. These swellings and bruises progress increasingly for the first two days. From day 3, begins to decline gradually. Early initiation arnica cream helps the bruises pass faster.

When resting after the procedure, keeping the head high with a few pillows helps to swell the edema less. In the first days after the procedure, there may be complaints about tension, stinging, burning the around eyes. These complaints can be resolved with simple painkillers.

From the 3rd day after the procedure, edema around the eye starts to decrease, and activities can be increased. On the 5th day after the procedure, bands and sutures around the eye are removed. A small band may be placed on the edge of the eye to stay another 5 days. On the same day, it’s allowed to rinse the eye contour and apply moisturizer to the eyelids. When performing these operations must be careful, and sharp movements should be avoided. When the bands and sutures around the eye are removed, purple areas that have begun to turn yellow and a limited swelling are natural. These decrease day by day, and can be hidden by light makeup. After the procedure, heavy sports, solarium, sauna, sun bath, steam bath etc. which increase edema should be avoided for 6 weeks.

In the first weeks, there may be tension, stinging and watering in the eyes due to wind and sun exposure. Sunglasses can be used to prevent these.

A thin, pink line on the upper eyelid fold, lower eyelid, under the eyelashes may be remarkable in the first weeks. In the corner of the eye, this situation may continue for some time longer in some cases. As the eyelid skin is too thin, after one year it’s quite difficult to distinguish the trail that will form. Again, in some cases, in situations that the outer margin of the eye needs to be tied up with a muscle sling, to be more apparent in the first week, the eyes may appear slanting. This situation decreases day by day.

What are the risks of eyelid surgery?

  • bleeding
  • infection
  • Blindness after blepharoplasty is an extremely small possibility. However, bleeding into the eye during surgery or after surgery may cause this. With early diagnosis and drainage, this problem can be accomplished.
  • The most feared and and relatively common complication is the downward retraction (ectropion) of the lower eyelid.

There may be changes in the appearance of the eyelid as a result of aging, sun exposure and some other conditions. Blepharoplasty surgery doesn’t stop aging, or provide constant tension of the eyelid area. New surgeries and treatments may be needed in the future in order to preserve the results of blepharoplasty surgery.

Fat Injection (Fat Transfer)

To ensure fullness in various parts of the face and body, it’s injecting fat into those areas. Cheeks, forehead, cheek-lip line, lips, chin, hips and crumpled areas on legs can be given as examples of areas where this application is made.

Fats are usually taken from the abdomen with special liposuction injectors. Waist and hip regions are also the areas where fat intake is frequent. Fats that are removed from the arms and legs of people who are planned to have liposuction, and even the fats taken from the under eye bags of patients that had eyelid surgery, can also be used for fat injection.

Fat injection is usually performed under local anesthesia. Sometimes, it can also be a stage of a surgical procedure that’s performed under general anesthesia. For example, plumping the cheeks with fat injection during a facelift operation… Removed fats, passing through some special processes (washing, centrifugation), are injected to the area by being transferred to an appropriate syringe. Since no incisions were made during these procedures, leaves no trace.

The more of the fat cells transferred to the new place live, the more permanent the fat injection will be. Due to some devices developed recently, removed fats can be transferred to the relevant region without being in contact with the air. Thus, as tissue loss is less, the duration of the fats retention is also prolonged.

The average 40% of the transferred fats can sustain their permanence after 6 months. So, in some cases repetition of the fat injection may be needed. There’s a greater chance of success in passive areas. For example, while fat injection in the cheek area is more successful, may be less prosperous in moving areas such as leg.

The most common application area of the fat injection is face. Some fat can be given to thicken the lips, to erase by filling the deep wrinkles at the corners of the mouth, to plump the cheekbones, or even to enlarge the lower jaw.

Fats are also often used to correct the shape of the hips, and to restore liposuction deformities that have previously been made and caused collapses. Collapses that occur after some accidents can be filled successfully with fat injection.

Breast augmentation with fat injection

For breast augmentation, fat injection can be performed in selected cases. Especially in women who do not want a bigger breast, if there’s sufficient resource in the waist or abdominal region, will work very well. Provides a very natural look. If another session is made after about 1 year, the result will be much better.

Fat injection takes about 30-60 minutes. Although it can be done under local anesthesia, its better to do it in the operating room environment. Thus, by using sedative drugs, the comfort of the patient can be improved. In addition, although it seems to be a simple method, as it occurs in many stages, the environment must be fully sterile.

What should be considered before the process?

For 10 days prior to the procedure, blood-thinning agents such as aspirin should be avoided. Also, if there’re significant discomforts in the patient’s history, and drugs used constantly, should certainly be reported to the doctor.

Even if the far injection is performed under general anesthesia, as it’s a process doesn’t require intervention at the hospital, the patient may go home the same day, a few hours after the procedure.

As soon as the fat injection is finished, cold application is started to reduce swelling in the transfer area. Cold application is done with the help of gels or bags filled with ice. Generally, it’s circulated through that area every 15 minutes per hour.

After the procedure, especially in the first two days, a swelling beyond the amount of fat injection in the treatment area is expected. This swelling will decrease over time. However, duration may be prolonged in injections made to the lip area, and the swelling may sometimes be excessive. The application area can be washed immediately the next day, and a moisturizing cream can be applied. After the 4th-5th days, a gentle massage to the application area helps to dissolve the swellings. Due to the increase of swelling, it’s necessary to avoid excessive temperature and steam.

Fat injection does not cause any discomfort in aspects that will affect the daily life of the person in general, except for the first few days. The first two days should be spent mostly resting due to the excess edema and bruising may occur in some areas.

Small bands placed at entry points for fat injection can be removed a few hours after the procedure or the next day. Antibiotic use is recommended for preventive purposes. This period is about 5 days.

Otoplasty / Pinnaplasty (Ear Surgery)

There can be found many congenital or acquired shape deformities at the ear that require ear aesthetic surgery. Congenitally complete or partial absence of the ear, periotic extra cartilaginous protrusions, the ear being puckered like wrinkled paper, pointed ears as Captain Spock (Stahl’s ear), or partial or total loss of ear in an accident… In society, we most commonly encounter the prominent ear deformity, which the ears are very apparent.

The so-called cup ears, that ears appearing overly large when viewed from the front, is a condition that can be frustrating to people of all ages. Men, women, without distinction, I have many patients applying with this problem. The ears get the adult size at 6. For this reason, as an exception to other aesthetic surgeries, it’s recommended that the otoplasty be performed preferably before primary school. So, one gets free from this problem before being exposed to the criticism of school friends. However, unfortunately, most of my patients are between 18 and 30. I think, in our country, only in those ages people have the chance to get rid of this discomfort by acting with their own decisions.

Otoplasty at younger ages are performed under general anesthesia. In adulthood, it can be done under local anesthesia, by using sedatives. If the patient is very anxious or young, general anesthesia is more appropriate. The surgery takes about 1.5 hours. Patients undergoing surgery with local anesthesia may go home the same day.

For 1-2 weeks after the surgery, numbness in the ear may continue. Therefore, I suggest not to use the hair-dryer during this time.

No stitch removal, the first check is on 4th day, and later on 15th day. On the 3rd day of ear surgery, can bathe with warm water. After a day’s rest, non-heavy workers can start working, using hair band. For those with heavier jobs, resting for 3 days will be healthier.

What should be considered after surgery?

A day after the ear surgery, changing the medical dressing, the ear is covered with a hair band that is not too tight. This band should be worn continuously for about 3 weeks. With this, the load on the sutures holding the cartilage that wants to return to its original state is reduced. The cartilage hardens and heals in the new position. Another function of the hair band is to reduce the edema, make the swelling better a little quicker, and prevent the ear from rubbing to the pillow or compulsion while sleeping in the first days after ear surgery.

The incision made for ear surgery is at the back of the earlap. The location of this scar isn’t visible when the head is viewed from behind after the ear surgery. However, it can be seen by pulling the ear out. This trace gradually decreases in 6-12 months, becomes almost uncertain.

What are the issues that may occur after ear surgery?

-bleeding

-hematoma

-infection

-asymmetry

-Sometimes, 4-5 months after the surgery, sutures that are put into the ear may try to get out by causing acne. By cleaning this area with dressing, sutures are removed.

Non-surgical aesthetic
